I'll do you one better. Here is a picture of it. I was going to use my 
scanner and scan it in but when I went to use it found that my scanner 
imaging light was burnt out. I guess it was better to find it now then 
in the middle of some production work.  :-\

Anyway the best I could do was to take a quick digital picture of it 
instead. It doesn't look the cleanest but you can see the screenshot and 
read all of the text.
