[Lazarus] Request for preview patch in #27764

Hello,

Can somebody please review and apply patch in bug report :

It is one new unit (Delphi compatible), which adds database login dialog 
(TForm descendant) and in its initialization section registers them to 
DB.LoginDialogExProc callback
